The management entity of the Michaels Organization, Interstate Realty Management Company, will provide and coordinate with service providers for the following free services:

	On-site Social Service Coordinator (licensed social worker)
	On-site Wellness Program (targeted specifically to frail elderly)
	Senior Companion Program through the Salvation Army
	Daily Living Assistance (in-home) through Visiting Homemaker Service
	Adult Senior Day Care Center
	Baldwin Foot and Ankle Doctor (monthly visits to Center)
	Medical Transportation Services for frail elderly - Lymari Invalid Coach
	Eye Care Screening through Phillips Eye Center
	Adult Medical Daycare (transportation to Center and two meals)
	On-site Lunch Program through Let's Celebrate faith based not-for-profit

Bon Secours and Canterbury Partnership for Care (formerly Franciscan Health Systems)

Assisted living services will be provided free of charge to support low-income seniors continued independent living/"aging in place":

	Case Management
	Nursing services
	Housekeeping assistance
	Meal Delivery and/or Assistance with Meal Preparation
	Food shopping
	Medication monitoring
	Assistance with Personal Care
	Counselling
	Transportation for doctor's appointments
	Recreational activities
	Assistance with financial planning
