1.    Relocation of Existing Residents

 A resident that has (or has a family member who has) a physical disability and is in need of an appropriately adapted apartment should contact the Site Manager to inform him/her of the disability and the need (using the Request for Relocation to Wheelchair Accessible or Hearing/Visually Adapted Apartment form which can obtained from the respective Site Manager).

 The Manager will request a "disability verification" which the resident can obtain from his/her doctor.

 Once the Manager has received the "disability verification" from the doctor, he/she will forward it to the Director of the Administration Department.  The Administration Department will maintain a list by date of information.  As the appropriate sized apartment becomes available, the disabled resident will be relocated.  The Administration Department will also notify the Applicant Selection and Accounts Receivable Department of changes.

2.    Placement of Disabled Residents from the JCHA Waiting List

If the handicapped apartment cannot accommodate existing residents with the appropriate disability, the Administration Department will inform the Applicant Selection Office of availability.  The Applicant Selection Office will assign an eligible, qualified applicant on the Public Housing Waiting List having a disability requiring the accessible features of the vacant apartment.
